Albertus Magnus Application Form

If applying to Albertus Magnus High School, we must be one of your three choices when registering to take the COOP exam.  You must have your current school complete and foward the ninth grade applicant record provided in the COOP exam registration packet. 

In addition, we require you to complete the attached application form and submit it to Albertus prior to December 1, 2008.
